Immigration Physical Exams

The United States Citizenship and Immigration Services (USCIS) requires a full physical exam to obtain green card or citizenship status.

Our office performs the exam, obtains the required lab tests, reviews and administers any required immunizations and completes your immigration paperwork. This form, I-693, will be completed by our supervising physician, a designated civil surgeon. We are here for you to ensure this process goes as smoothly and stress-free as possible.

To prepare for your immigration physical, please bring the following items to your appointment.

1) A printed I-693 form. Before printing, please complete the sections on page 1 and the interpreter’s information on pages 2-3, if applicable. Please do not sign or date anywhere. If you were not assigned an A-number, leave it blank. The form can be found and downloaded at: https://www.uscis.gov/sites/default/files/document/forms/i-693.pdf

2) A state-issued license, passport, or any other legal form of identification.

3) All immunization records.
